Waiver Release Indemnity and Assumption of Risk

As a part of the reservation and registration for the Dive Trip, certain forms will need to be completed and returned to me or brought with you on the trip and provided to Buddy Dive Resort.  Currently, the Waiver Release Indemnity and Assumption of Risk form is needed to be submitted to me as part of the reservation and deposit for the trip.  If you have not already completed this Waiver, please complete it and return it to me as soon as possible.  The Waiver, along with the Trip Registration Form and the Deposit of $200.00 per person will confirm your spot on the trip.  Please email a completed Waiver in pdf format and send it to yoscuba@yoscuba.com or mail it to me at:
